Looks like a humerus with the same fixation devices in the X-ray you posted, but fracture now completely separated on the second X-ray and the plate obviously broken.

Must have been some window...
The window was the straw that broke the camel's back. Honda is trying to deflect cause/effect away from his coming back too early to make it seem that the titanium plate broke from fresh with him simply opening a window. The plate broke after repeated stress from his premature comeback.

To apply a plate you need to strip the periosteum from the bone. The periosteum is what is responsible for the generation of callus and consequent healing. Each time they fiddle with with the injury there is the risk of non-union. What's the point of a beautiful plating with non-union? All of this adds up to not upsetting the apple cart any further. Ensure union before moving forward. A very sensible approach after a regrettable decision to allow him to race.

I don't know if there was an operation that went badly. It's more likely that what they did in the second operation was all that they could do to ensure the greatest likelihood of bone union/healing. What we do know is that it was a poor decision to allow him to race. I was shocked to see him there to be honest. I would have been shocked to hear that the plating stood up to the punishment of what he did in the buildup and during the race weekend. It's bone and not a plate that heals continuously and especially during a night's sleep. He likely just went about his business with the plating as recommended and it finally gave up the ghost while opening a window.
